According to this data, It sounds to me like their has been a total break-down in the safety training program, I don't know what type of safety processes, they currently have in place, if any, but its obviously not working.

I think they shoud first perform a safety survey of the construction workers, to assess their overall attitudes regarding personal safety. And then implement a BBS, bahavior-based safety program, which could eventually, be totally turned over to the workers thereby making the people whom are most affected by a safe working environment, responsible for maintaining it, in other word, they could start policing themselves. There's a simple to implement plan called, DO IT, which stands for, Define, Observe, Intervene and Test.

Firstly, you difine and target the a specific hazard, which in this case would be fall safety, obviously. Secondly, you observe the workers or have them observe one another at work looking for at-risk vs. safe behavior and document both. Thirdly, you intervene with coaching, positive reenforcement or disciplinary action, if required. And lastly you test the progress by observation and worker feedback, to see if there is actually a reduction in the targeted at-risk behavior.

And if you don't see any changes to the targeted at-risk behavior during to teating phase, simply repeat the process. By changing the safety environment and behavior of the workers, you'll also affect their attitudes as well.
