Longevity Milestone: First Human Test Underway in Breakthrough Clinical Trial of Age Reversal

In what may be a monumental leap for regenerative medicine, scientists say they have officially initiated the world’s first human clinical trial for partial cellular reprogramming to reverse human aging. Boston-based biotechnology firm Life Biosciences on Tuesday announced that it has successfully dosed the first participant in its unprecedented study to increase longevity by safely reversing the physical hallmarks of aging. “This is an important moment for Life Bio and for the field of aging biology,” the co-founder of Life Biosciences and a professor of genetics at Harvard Medical School, David Sinclair, said in a statement. “Our research has suggested that aging is driven in large part by the loss of epigenetic information, not irreversible damage. This clinical study represents the first opportunity to test whether restoring that information can ameliorate human disease.”The trial centers around an experimental gene therapy known as ER-100, currently being administered to patients suffering from severe age-related optic nerve damage, including glaucoma. By delivering modified reprogramming factors directly into the eye, the therapy attempts to rejuvenate damaged cells and restore lost visual function. Unlike previous treatments that manage only symptoms, ER-100 aims to fundamentally rewind the biological clock of mature optic nerve cells while preserving their specialized function.The underlying technology relies on a concept called “partial reprogramming.” Inspired by the Nobel Prize-winning discovery of Yamanaka factors — four proteins capable of reverting adult cells back into stem cells, turning back the biological clock — the trial uses a refined approach. Fully resetting cells carries severe risks such as uncontrolled growth and tumor formation. To bypass this, ER-100 only partially rewinds the cellular aging process. Patients receive a single gene therapy injection, followed by a specific course of antibiotics that acts as a chemical switch to activate the rejuvenating genes.Because the eye is relatively isolated from the rest of the human body, it serves as an ideal testing ground to monitor the therapy’s safety and effectiveness. Researchers will closely observe the initial group of fewer than 20 patients across clinics at Boston, New York, Los Angeles, and Charleston. Though just one human subject is being treated so far, Mr. Sinclair has long been searching for the proverbial fountain of youth. His previous research demonstrated repairs to the optic nerve of a mouse caused by age-related damage. He also presented data showing restoration of electrical activity in the retina of a monkey. The findings served as the foundation for the human trial. Mr. Sinclair’s progress, however, is not without detractors. In 2024, several scientists at the Academy for Health and Lifespan Research, which Mr. Sinclair co-founded and where he served as president, resigned in protest after what they called overzealous and unproven claims that a supplement Mr. Sinclair developed at one of his other companies, Animal Bioscience, reverses aging in dogs. The scientists who challenged the assertions reportedly said they could not agree on how to define “reverse aging” much less how to measure it. The scientists also expressed concern that Mr. Sinclair was profiting by peddling unconfirmed breakthroughs to consumers. Mr. Sinclair resigned as president while the company revised the language of the supplement’s press release. Nonetheless, in the competitive and profitable field of longevity, Mr. Sinclair is one scientist who is closely followed. A biomedical scientist not associated with his study, Avi Roy, said Mr. Sinclair’s latest research could prove that “aging is information loss. The youthful settings are still in the cell, buried under noise, and these factors let it read the backup.”If this Phase 1 trial does delay age-related cellular decline or prove that the ER-100 can be safely reversed without triggering uncontrollable cell division, the milestone could pave the way for revolutionary treatments targeting a wide array of other age-related conditions, including arthritis, cardiovascular disease, and Alzheimer’s.
