Countdown Has Begun as America Braces To Lose Its Measles-Free Status in November

More than 3,200 cases, three measles-related deaths in Pennsylvania, and a kindergarten vaccination rate below 93 percent have public health officials bracing for the end of a designation the country has held since 2000.That possibility has led to significant squabbling among the states and federal government. Pennsylvania’s Department of Health confirmed late last month that two unvaccinated Lancaster County residents, both infants, had died with measles, the state’s first deaths from the virus in 35 years. A third Pennsylvanian, a 40-year-old woman from Jefferson County north of Pittsburgh, died September 12.Governor Shapiro called the losses entirely preventable, and one of the infant’s deaths has since become a source of dispute. While the health department classifies it as “measles-associated,” meaning laboratory evidence of the virus was present, the Lancaster County coroner, Stephen Diamantoni, said the male baby died shortly after birth of an internal hemorrhage and a ruptured spleen.Dr. Diamontoni confirmed finding traces of the measles virus in the infant’s lung tissue. However, he has since clarified that measles was a “significant condition” at time of death, not a contributing cause.The other, a six-week-old girl born with Amish lethal microcephaly — a genetic condition causing an underdeveloped brain, with affected children typically living six months or less — did die in part due to measles, Dr. Diamantoni confirmed, as children with the condition are highly susceptible to infections.Nationally, three people died of measles in 2025, including two children in Texas and an adult in New Mexico. But the three Pennsylvania deaths are a flashpoint in a dispute between state and federal health officials who have clashed publicly over how measles-associated deaths should be counted and reported, even as the broader surge rewrites the record books.The United States had confirmed 3,471 measles cases as of the CDC’s most recent update on September 18 — already the country’s highest annual total in more than 35 years. Of those, 3,454 were reported across 47 U.S. jurisdictions. The remaining 17 cases were among international visitors. The CDC has logged 39 outbreaks this year, compared with 48 in 2025. Case counts are updated weekly so the figures are a snapshot rather than a final tally. Behind those numbers sits a bureaucratic deadline that could formally end a public health milestone the country has held for a quarter century. The New York Sun reported in January that the United States and Mexico were already flirting with the threshold after the Pan American Health Organization, the body responsible for certifying elimination status across the Americas, removed Canada’s measles-elimination status last November.“PAHO will determine in November whether the U.S. has maintained or lost its elimination status. Regardless of that outcome, the U.S. has a large measles problem that calls for immediate action,” the professor of public health at the University of North Carolina’s Gillings School of Global Public Health who chairs the national committee reviewing the case, Noel Brewer, tells The New York Sun.The United States earned its measles-free label when health officials certified that the virus had stopped circulating on its own. The designation allows for the contagion to resurface — brought in by a traveler, sparking an outbreak that infected dozens or even hundreds of people — however, as long as no single chain of infection ran longer than 12 months, the country kept its status.The Pan American Health Organization was originally scheduled to review the American case in mid-April. That review was pushed to November as health officials work through genomic sequencing data to determine whether the outbreaks recorded since January 2025 trace back to a single, unbroken chain of transmission or represent a string of separate importation events.Dr. Brewer’s committee met in mid-August and is expected to forward its findings to the Pan American body ahead of the November decision.The genetic detective work matters because elimination status hinges entirely on a continuity question. Two viral genotypes have circulated in the current outbreaks, and a strain identified in West Texas, Utah, and South Carolina has been the dominant thread linking cases across state lines.The Centers for Disease Control and Prevention enlisted the Broad Institute in December to help sequence the outbreak’s genome, while Utah’s own public health lab has moved faster on its own, sharing hundreds of measles genomes ahead of the federal agency.If investigators find that transmission never really stopped since the West Texas outbreak began in January 2025, the designation lapses. Add up 2025 and 2026, and the case count already tops 5,500.“It is very likely that the U.S. will lose its elimination status. We have continued to break records. Our numbers indicate that there is ongoing local transmission,” a professor of global health and epidemiology at George Mason University, Amira Roess, tells the Sun.The Vaccination Gap Driving the NumbersOne denominator in this year’s data more than any other is vaccination status. Ninety-four percent of confirmed 2026 cases have involved people who were unvaccinated or whose vaccination status was unknown, a trend nearly identical to 2025. Roughly two-thirds of this year’s patients are 19 or younger.This spread may be traced to grade school. During the 2025 to 2026 school year, only 92.4 percent of kindergarteners received the combined measles-mumps-rubella vaccine. Researchers say communities need 95 percent protection to stay healthy. That rate was sitting at 95.2 percent six years ago. Today’s shortfall has left around 280,000 kindergartners without documented measles protection this school year.Vaccine exemptions climbed alongside the drop, reaching a record 4.2 percent of kindergartners nationally, up from 3.6 percent the prior year, with two dozen states now recording exemption rates above five percent.“It has become increasingly common for parents not to turn in paperwork to show measles vaccination of their children entering kindergarten. This is not the same as vaccination rates decreasing,” Dr. Brewer explains. “That said, it is true that vaccination rates have slid a small amount in recent years.”From his purview, the reason for these changes is unclear, “but may include anti-vaccine activism and more vaccine skepticism in a small number of parents.”“Measles vaccination remains one of the most widely supported health behaviors in the U.S.,” Dr. Brewer says.The debate has taken on a new political edge this year. President Trump signed an executive order in August, standing alongside Secretary of Health and Human Services Robert F. Kennedy Jr., that calls for splitting the combined MMR shot into three separate vaccines and for children to receive fewer immunizations at each visit. Federal health guidance has previously said those changes carry no proven benefit. Several medical groups warn the split could leave children unprotected for longer between doses. Parents supportive of the vaccine say splitting the shot makes it more onerous and more expensive to take children to the doctor, increasing the trend toward lower vaccination rates.The slide in coverage, however, predates that fight by years. Kindergarten MMR rates have fallen every school year since the 2019-2020 term, well before Mr. Trump’s second round in office began, and even earlier than disputes over the Covid vaccine raised skepticism about overall vaccine safety and efficacy.Public health researchers point to a mix of causes behind the broader shift. They include pandemic-era distrust of public institutions, a resurgence of long-discredited claims linking vaccines to autism, and a growing willingness among some parents to claim religious or personal-belief exemptions rather than catch up on missed doses.“We have been on a trajectory of increased vaccine exemptions, especially religious and personal belief exemptions, for over two decades. It will take time to reverse this trend,” Dr. Roess says, underscoring that halting the current outbreak season will require action on two very different timelines.“We would need local interventions to more quickly identify cases and implement quarantine and isolation measures. We would also need long-term interventions to improve vaccination rates,” she adds, pointing out that local response is already under strain.“Communication is key. Whenever a case is suspected, it will be critical to quickly investigate, quarantine, and isolate to avoid further spread,” Dr. Roess says. “This is very difficult to do given the ever-dwindling public health resources needed.”What Losing the Status Would MeanMeasles elimination is a technical designation, not a guarantee that an outbreak will never happen. The United States came close to losing its designation once before, in 2019, when an outbreak centered at New York hit nearly 1,300 cases across 31 states. Health officials managed to shut down transmission just before the 12-month clock ran out.A loss would not result in new federal restrictions or emergency powers. Instead, it would mean measles has quietly become endemic in the United States again, rather than a condition that occasionally slips in from abroad.The United Kingdom also lost its status once before, in 2019, and spent years earning it back. It lost it again in January.If the dubious distinction is conferred, it would arrive at a moment when global measles activity is elevated among countries, including India, Angola, and Indonesia, raising the odds that international travelers continue seeding new domestic clusters even after existing chains are broken.For now, the outbreaks driving this year’s numbers are still active in Pennsylvania and several other states, and the Centers for Disease Control and Prevention updates its national count weekly. The verification committee’s findings, and the Pan American Health Organization’s ultimate determination in November, will decide whether the elimination label America has carried since 2000 survives the year that broke every recent measles record to get there.A professor of leadership at the City College of New York, Dave Chokshi, characterizes the potential status loss as a chilling step in the wrong direction “The United States is on track to lose measles elimination status, a hard-won public health milestone achieved more than two decades ago,” Dr. Chokshi says. “The loss marks a turning point. It is an acknowledgement that a disease we know how to prevent has been allowed to return, leaving a path of harm to children, families, communities, and economies in its wake.”While measles isn’t a death sentence for most, it is nothing to sneeze at. The CDC notes that historically one to two of every 1,000 children who contracted measles died from respiratory and neurologic complications. By comparison, 1.11 per 1,000 Americans, mostly the elderly, died of Covid-related complications at the peak of the pandemic in 2021, according to the CDC.
