Doctor Submitted Bogus Botox Medicare Claims To Scam Millions in Real Estate and Make Bizarre Purchases

A Los Angeles County doctor defrauded Medicare of $45 million by administering Botox injections for patients she had falsely diagnosed with conditions and used the proceeds of her scam to fund a luxury lifestyle that included vacations to Maui, a Tesla Cybertruck, and a 17th-century crossbow worth $12,000.Violetta Mailyan, a 45-year-old owner and operator of the Healthy Way Medical Center, was convicted in the Central District of California court in May of filing more than $45 million in claims for Botox treatments to Medicare using fraudulent medical records, of which she received more than $32 million in payments from 2019 to 2025. Prosecutors said she had collected more in Medicare Botox reimbursements than any other provider in the United States.FBI assistant director in charge of the Los Angeles Field Office, Patrick Grandy, told the Los Angeles Times Mailyan committed “the largest Botox fraud scheme in the United States.”Medicare typically covers Botox when used as a treatment for chronic migraines, among other serious ailments. It does not cover Botox when used to smooth wrinkles. Maliyan still administered Botox for medically unnecessary reasons and changed the medical records —using thirteen different diagnosis codes for migraines — to fraudulently classify the treatments for covered conditions. Federal prosecutors said Mailyan received Medicare payments after billing for thousands of injections that were never provided or occurred on dates when she was actually away on a luxury trip to Cabo, Maui, or New York. In one instance, she submitted a bogus claim for a patient who was in federal prison at the dates of treatment. Mailyan then used the proceeds of her scheme to purchase fine art, like a $12,000 17th-century Florentine Mannerist crossbow and a $3,000 painting of Ludwig I, the crown prince of Bavaria.The Justice Department’s Health Care Fraud Section’s Data Analytics Team noticed Mailyan received, at the time, more than $24 million in the previous four years. It was an amount that was six times higher than that of the next highest group of providers, all of whom were neurologists. Among her assets seized by federal authorities are a Tesla Cybertruck, brokerage accounts worth more than $7 million, and four properties in Surfside and Glendale, California, that are valued at a combined $7.3 million. On her Instagram, Mailyan identified herself as a “Botox filler nonsurgical nose cosmetic doctor,” and in one video, she thanks her patients while holding a glass “platinum provider” award. Mailyan was convicted of nine counts of wire fraud and three counts of obstruction. She is facing up to 20 years in prison for each count of wire fraud and five years for each obstruction count. A sentencing hearing has been set for September 10.
