How Winner of Refugee Award Came To Be Charged in Medicaid Fraud Ring Linked to Sex Trafficking

Salman Ahmed Elmi’s passport is gone, and so, for now, is his freedom to leave Minnesota. He surrendered on August 17, walking out of a Ramsey County courtroom on bail after prosecutors charged him with eight felony counts — three of aiding and abetting theft by swindle, five of aiding and abetting theft by false representation.At the center of it is an alleged scheme that prosecutors trace back to 2023 worth at least $4.5 million. The 30-year-old St. Paul man is due back in court October 4.So, how does a state hold someone up as its model immigrant success story, then charge him with looting the very program meant to help people like him?Five years ago, Mr. Elmi stood among Minnesota’s honorees for the Entrepreneurship Award under the state’s Outstanding Refugee Awards, and the state held him up as proof that the American dream was achievable. Now prosecutors say his company was a vehicle for stealing from a taxpayer-funded health program meant to serve the state’s most vulnerable residents, and that the case intersects with an unrelated sex-trafficking prosecution that has ensnared a sitting county prosecutor.The playbook is a familiar one to those who study how government health programs end up being exploited.“There is a general presumption that any qualified medical provider is entitled to offer services under major government programs, like Medicaid and Medicare, unless there is a specific and proven history of fraudulent conduct,” a professor of the practice of public management at the Harvard Kennedy School, Malcolm Sparrow, tells The New York Sun. “It’s a pay-on-trust system, and the simple rule for crooks is, bill your lies correctly.”A Celebrated Immigrant Story UnravelsProsecutors allege that Mr. Elmi and co-conspirators created four fake agencies, including his company Reva Health, using forged documents to bill Minnesota’s Medicaid system for services that were never delivered.Reva Health, based in Golden Valley, billed the state for more than $1 million in adult rehabilitative mental health services that Mr. Elmi allegedly never provided or that were never eligible for reimbursement, according to the complaint. The Department of Human Services halted payments to Reva Health and several related businesses in February.The notion that Minnesota once celebrated Mr. Elmi as an entrepreneurial success only sharpens the contrast with what prosecutors now allege.Archived Department of Human Services records show Mr. Elmi telling the department he started his business “to help inspire other people.” He also built a public profile around Tavolo, an artificial intelligence marketing app for restaurants that drew investment from Shark Tank’s Kevin O’Leary. A DHS spokesman called Mr. Elmi’s alleged conduct disturbing and said it does not reflect what the awards are meant to stand for.From Commercial Sex Houses to Assisted Living HomesOne of Mr. Elmi’s alleged co-conspirators is Andrea Sampson,who spent nearly a decade as an assistant county attorney. She was charged last month with 13 counts of promoting prostitution and 13 counts of generating profits from commercial prostitution. Investigators say some of those properties, spread across Minneapolis, St. Louis Park, and Bloomington, were later repurposed as licensed care sites. With co-defendant Frank Reeves, whom prosecutors identified as the operation’s ringleader, Ms. Sampson allegedly co-owned four care companies: Reva Health; Reva Autism; Helpful Housing; and Homefelt Assisted Living.Those four businesses billed Medicaid roughly $4.65 million between May 2023 and February 2026. Investigators have flagged about $2.5 million of that as potentially fraudulent.Ms. Sampson, however, never left her day job. She worked full time at the Anoka County Attorney’s Office through March 2026, the same stretch prosecutors say the trafficking-and-fraud operation was active. The office hasn’t said publicly whether any of her casework overlapped with the allegations against her.The four agencies at the center of the case — Reva Health, Reva Autism, Helpful Housing, and Homefelt Assisted Living — share overlapping ownership: Prosecutors allege that Mr. Elmi, Mr. Reeves, Ms. Sampson, and a fourth defendant, Viktoriy aKomonash, operated the entities together, which is how a Medicaid billing case in Ramsey County came to intersect with a sex-trafficking indictment in Hennepin County.The convergence between a Medicaid fraud ring and other criminal enterprises is not as unusual as it might seem.“This particular link is not so common, but organizations set up to engage in Medicaid fraud are frequently involved in other types of crime as well, including bribery, extortion, money laundering, and support of terrorism,” Mr. Sparrow explains.The weak point in the system, according to a second expert, comes early in the process, when a provider first gets approved to bill the program.“It shows that provider screening is often little more than a paperwork exercise. Fraud networks routinely use the same shell companies, straw owners, and real estate holdings across multiple criminal enterprises,” the managing director of Nestpoint Associates, John Thomas, tells the Sun.A State Already Drowning in FraudThe Elmi and Sampson cases did not surface in isolation. Rather, they are the latest entries in a Medicaid fraud scandal that federal officials have called the worst in the country. State records show autism providers alone billed Minnesota’s Medicaid program $1.6 billion between 2018 and 2025, with only 56 percent of those claims ultimately approved. Investigators believe more than half of that may have been fraudulent.The first assistant United States attorney in the state, Joseph Thompson, said in December that as much as half of the roughly $18 billion in Medicaid claims paid out since 2018 across 14 programs flagged as high-risk could be fraudulent, an estimate the Department of Human Services’ inspector general, James Clark, has called shocking.Federal prosecutors brought a string of new cases last winter targeting the state’s autism-therapy and Housing Stabilization Services programs. Regulators shuttered the housing program entirely after its spending ballooned from a projected $2.6 million a year to more than $100 million.For one longtime observer of Minnesota’s government, the scandal runs deeper than any single scheme.“Over a 40- or 50-year period there has been a bipartisan effort to dismantle many of the administrative checks designed to prevent fraud and abuse. On the Democratic side, significantly more money has gone to social services. Still, comparable resources have not been devoted to audits and oversight,” a professor of political science and law at Hamline University, David Schultz, tells the Sun.“On the Republican side, efforts to curtail government spending have encouraged significant contracting out, yet adequate resources were not provided for oversight of those private contractors.”Some in Minnesota have tried to pin the fraud epidemic on the state’s Somali population, which Mr. Schultz rejects outright.“That is an unfair racial or ethnic explanation of what is fundamentally a political and institutional problem,” he says, noting that the intersection between Medicaid fraud and sex trafficking uncovered in the Sampson case is a genuine outlier. “I have never previously encountered this particular intersection.”However a Republican state senator, Michael Kreun, has argued that Minnesota has known about its fraud epidemic for a long time and simply failed to contain it, calling the state the epicenter of fraud in the country. Ms. Sampson’s case moves forward in Hennepin County, and Minnesota’s Medicaid Fraud Control Unit says the scope of what it has uncovered is likely far from complete.The Sun has followed this scandal since it first reached Capitol Hill, when lawmakers began pressing Governor Tim Walz on what he knew and when he knew it. That was in December. By January, the problem no longer looked like a uniquely Minnesota story.Officials elsewhere started asking whether the same gaps in verification existed in their own Medicaid programs. Washington moved next. The Trump administration froze child care and family cash assistance payments to five states in mid-January, Minnesota among them, over concerns that taxpayer money was being misused.For Mr. Thomas, the fix must start before any money goes out the door.“Real deterrence requires upfront verification with teeth, such as mandatory beneficial ownership disclosure, cross-checks against criminal databases before any billing begins, and rapid payment freezes when red flags appear. These schemes will keep proliferating.”Yet, Mr. Schultz views the current political response, including audit pledges from both major candidates in the governor’s race, as being a step behind.“That is the proverbial closing of the barn door after the horse has gotten out,” he says. “We need stronger whistleblower protections and repeated independent outside audits.”
