James Van Der Beek’s Untimely Death Puts Renewed Focus on the Colorectal Cancer Crisis Stalking Young Americans

James Van Der Beek, the actor who became a generational icon through his starring role in “Dawson’s Creek,” has died at 48 following a battle with colorectal cancer. His family announced that he passed peacefully, noting he met his final days “with courage, faith, and grace.” Yet Mr. Van Der Beek’s death underscores a disturbing trend that has alarmed oncologists and public health officials: colorectal cancer rates among younger adults have been rising sharply over the past two decades. While this disease was once considered primarily a condition affecting those over 50, researchers are now documenting increasing incidence among people in their 30s and 40s, prompting urgent investigations into potential environmental, dietary, and lifestyle factors that may be driving this alarming shift.Behind the statistics are countless individuals who never imagined they’d face this diagnosis so young.When Nadja Atwal first noticed a change in her body odor, she brushed it off as hormonal fluctuations. The New York City public relations professional, television host and mother to two young sons felt energetic and healthy, and such changes, while odd, seemed nothing to worry about.It wasn’t a stronger version of her normal scent; it was something altogether different, more intense and distinct. She didn’t realize it was an early warning sign.Only after her colorectal cancer diagnosis in 2021 did the pattern become clear.“I’m shocked that it’s not in the protocol, that this isn’t something people tell you to look for,” Ms. Atwal tells The New York Sun.Human-detectable body odor changes aren’t established as a standard early warning in official literature. Ms. Atwal, however, is among a growing number of Americans under 50 confronting a disease once considered exclusive to the elderly. Recently, research from the American Cancer Society confirmed what doctors have been dreading: colorectal cancer is now the leading cancer killer of Americans under 50, surpassing lung and breast cancer after climbing steadily from fifth place in the early 1990s.While overall cancer deaths in this age group have plummeted by 44 percent over three decades, colorectal cancer deaths have moved in the opposite direction, rising roughly one percent annually since 2005.Three out of four young patients receive their diagnosis only after the cancer has reached advanced stages, when five-year survival rates plunge from 90 percent for localized disease to just 14 percent for stage IV cancer. Yet now, after years of searching for answers, scientists believe they have identified at least one significant culprit hiding in plain sight: a bacterial toxin that damages DNA during childhood, setting a deadly clock ticking decades before symptoms appear.The Science of ScentMs. Atwal’s observation about changes in body odor aligns with emerging research on volatile organic compounds (VOCs), gaseous carbon-based chemicals emitted by the human body that reflect metabolic processes. Cancer cells produce distinct VOCs through altered metabolism, oxidative stress, and cell death, creating what researchers call “cancer odor.” These compounds, detected in breath, urine, sweat, and on the skin, can change a person’s typical scent in subtle but detectable ways.Studies have also shown that trained dogs can detect colorectal cancer via scent, with 91 percent sensitivity and 99 percent specificity, by identifying VOC signatures. Research at the University of Pennsylvania’s Working Dog Center and other institutions demonstrates that cancer produces chemical changes detectable at concentrations as low as one part per trillion — the equivalent of one teaspoon of sugar in two Olympic-sized swimming pools.While body odor changes signal cancer’s presence, scientists have been working to uncover what triggers the disease in the first place.The breakthrough explaining rising rates came from an international team led by University of California, San Diego bioengineers who analyzed tissue samples from 981 colorectal cancer patients across 11 countries. Their findings, published in Nature last April, revealed that a toxin called colibactin leaves distinctive mutation patterns in human DNA. Patients diagnosed before age 40 were more than 3 times as likely to harbor these mutations as those over 70.Colibactin is produced by certain strains of Escherichia coli and other gut bacteria as a chemical weapon to defend against microbial competitors. Unlike the E. coli strains that cause food poisoning, these colibactin-producing bacteria colonize the intestines silently, without symptoms, while their toxin attacks the DNA of colon cells.The damage creates interstrand cross-links that act like molecular glue, binding DNA strands together in ways that are difficult for cells to repair.What makes the discovery particularly alarming is the timing of exposure. Using molecular clock analysis, researchers determined that these DNA mutations are likely to occur within the first decade of life.Children can acquire colibactin-producing bacteria through contaminated water, unwashed hands, or contact with unclean surfaces. Once established in the gut, these bacterial strains can persist for years, continuously producing their genotoxic payload.Lead researcher Ludmil Alexandrov describes the mechanism in stark terms: if someone acquires one of these driver mutations by age five or ten, they could be decades ahead of schedule for developing colorectal cancer, potentially facing diagnosis at 40 instead of 60. The toxin appears to account for approximately 15 percent of the earliest genetic alterations that directly promote cancer development in colorectal tissue.“It is very difficult to precisely eliminate a specific bacterial species within a complex community,” microbiome researcher at the University of Florida, Christian Jobin, tells The Sun. “At the preclinical level, it is possible to target E. coli pks using a cocktail of bacteriophages.”Mr. Jobin also points to emerging research showing that a bacterial strain producing a colibactin antitoxin, clbS, may offer protection. However, these interventions are not yet ready for clinical use.“It is important to factor in diet when thinking about the relationship between microbiota and early-onset colorectal cancer, he says. His research demonstrates that dietary fiber can mitigate the genotoxic effects of colibactin-producing E. coli.Currently, no commercial test exists to identify colibactin-producing bacteria in the gut or detect colibactin-related mutations in patients, leaving exposure invisible until DNA damage has already occurred. Researchers are developing chemiluminescent probes and PCR-based biosensors to detect colibactin-producing bacteria in stool samples. Yet, these technologies are still in development and not yet available for clinical use.The Diagnostic GauntletThe research arrives as young patients continue to face a brutal reality: their symptoms are routinely dismissed. A 2024 analysis of 81 studies involving nearly 25 million patients found that blood in the stool was the initial symptom in 45 percent of early-onset cases, followed by abdominal pain in 40 percent and altered bowel habits in 27 percent. Yet the time from symptom presentation to diagnosis often stretched up to 6 months, with delays common among younger patients.“There are several common threads in the narrative I’m hearing from doctors,” Ms. Atwal points out. “One is the food, another is the water, microplastics, and also the overuse of antibiotics.” Antibiotic use in early childhood has emerged as a particular concern. Research has linked early-life antibiotic exposure to increased colorectal cancer risk later in life, as antibiotics kill both harmful and beneficial gut bacteria indiscriminately.This disruption can eliminate protective bacterial strains that usually compete with and suppress potentially dangerous bacteria, including colibactin-producing E. coli, allowing harmful strains to colonize the gut and persist for years.“That’s why I always say and always do it when the kids are on antibiotics, in between taking the antibiotic, always give them probiotics,” Ms. Atwal underscores.Environmental Triggers Remain ElusiveNot every colorectal cancer patient carries the colibactin mutation signature, however, and researchers acknowledge that the toxin cannot be the sole explanation for rising rates. Studies show colibactin-related mutations appear in approximately 7.5 to 12 percent of colorectal cancer cases, though they are significantly enriched in early-onset patients.Mr. Jobin acknowledges that this toxin is likely one of many gut health problems that, combined, can push some people toward cancer more quickly. Several environmental factors may also influence whether colibactin-producing bacteria thrive in the gut. Epidemiology data suggest that sulfur-metabolizing bacteria are associated with early-onset colorectal cancer, Mr. Jobin notes.Mr. Alexandrov’s team is investigating whether the rise in cesarean births has altered how babies’ gut bacteria develop, potentially allowing more harmful strains to colonize the gut. Decreased breastfeeding, increased antibiotic use in early childhood, and diets high in ultra-processed foods could all disrupt gut health in ways that favor colibactin producers.Other bacteria have also drawn scrutiny. Fusobacterium nucleatum has been implicated in promoting tumor proliferation and immune evasion, while Bacteroides fragilis appears to enhance DNA damage when it co-occurs with colibactin-producing E. coli in patients with hereditary cancer syndromes. The picture emerging is one of complex microbial interactions rather than a single bacterial culprit.Screening and SurveillanceYet even when young patients do develop symptoms and seek medical care, they face another obstacle: diagnostic dismissal.Hemorrhoids have become the default diagnosis, delaying detection. A survey by the Colorectal Cancer Alliance found that 43 percent of young patient respondents were initially misdiagnosed, with hemorrhoids cited alone or in combination with other conditions.By the time many patients receive accurate diagnoses, their cancers have already progressed to advanced stages; a problem that screening guidelines are now attempting to address.Current guidelines recommend colorectal cancer screening beginning at age 45, lowered from age 50 in 2021 after the United States Preventive Services Task Force recognized the dramatic rise in younger cases. About 20 percent of colorectal cancer cases are now diagnosed in people 54 or younger, double the proportion seen in 1995.Colonoscopy remains the gold standard for detection, capable of identifying cancers at their earliest stages and removing precancerous polyps before they progress to malignancy. Stool-based tests offer alternatives for those unable or unwilling to undergo colonoscopy, though specialists emphasize that any positive result or persistent symptoms should prompt immediate colonoscopy.For Ms. Atwal, the unusual scent returned to normal after surgery and chemotherapy. Still, when it reemerged years later following her all-clear, she knew immediately the cancer had returned or had never entirely left.“I noticed before I was diagnosed the first time, and I told them that I now, retrospectively, my body odor changed because then, after treatment, it went back to normal,” she says. “So when the cancer was growing again, because it never was fully eradicated, I noticed the body smell again.”She insisted her oncologist investigate immediately.“I told my oncologist, we need to get cracking right away. I don’t care what the test is showing,” Ms. Atwal says.When a CT scan showed something suspicious but inconclusive, her doctor ordered a blood test from Natera. The company’s Signatera test is “custom-built for each patient from their tumor, that can detect tiny quantities of tumor DNA in the bloodstream and determine whether a patient’s cancer has come back or if they are responding to treatment,” a Natera spokesperson tells the Sun. The test “can detect recurrence many months and up to two years before traditional scans.”“I think Natera is the single game changer, life saver in cancer right now,” Ms. Atwal says. Her test came back positive in 2023, detecting circulating tumor DNA despite inconclusive imaging. The company even shipped the test kit to her while she was visiting family in Germany.“I went to my doctor for a blood draw. They had everything prepared, shipped it back, and within the week, you have your result,” she says. “Why isn’t something simple like this available to everybody, all around the world at all times?”The company reports that Signatera is now “used by more than 50 percent of oncologists nationwide and supported by over 140 published clinical studies.”For younger patients, the spokesperson notes, the test’s ability to guide treatment decisions is “especially important for younger patients who may live decades beyond their diagnosis,” potentially helping them “safely forgo treatment like chemotherapy or benefit from less toxic therapy regimens.”The Human CostWhile answers about absolute prevention are elusive, the human cost continues to mount. Colorectal cancer is now not only the deadliest cancer among young men but also the second deadliest among young women. The disease interrupts careers, damages finances, and forces young patients to make rapid decisions about fertility preservation before treatment begins. Yet only 24 percent of young patients report being counseled about fertility preservation, according to the Colorectal Cancer Alliance survey.Ms. Atwal urges patients to become their own advocates.For now, awareness is the most powerful tool available. Blood in the stool, persistent abdominal pain, unexplained weight loss, changes in bowel habits, unusual body odor changes, or iron deficiency anemia should prompt immediate colonoscopy regardless of age. The disease that once belonged to the elderly has become a threat to Gen Z, and dismissing symptoms as stress or hemorrhoids has proven to be a mistake that costs lives.“I already have borrowed time,” Ms. Atwal, who is now all clear and feeling top-notch, adds. “If I had been diagnosed a few months later, I would have been a death sentence. So I feel lucky.”
