Putin’s Ever-Changing Face Fuels Questions About What the Kremlin Is Hiding

The cheeks gave him away first: pillowed and oddly taut, crowding a pair of narrowed, tired eyes beneath a forehead implausibly smooth for a man of 73. President Vladimir Putin marked Russia’s most sacred national holiday last month with a face that set the internet ablaze.He appeared noticeably swollen at the May 9 Victory Day parade on Red Square, an event so scaled down that no tanks rolled past the reviewing stand for the first time in years, and within hours photographs of his puffy features and gaunt expression had spread across social media. One Ukrainian commentator, sharing an unflattering close-up of the bulging cheeks, quipped that Western sanctions had finally reached the president’s Botox supply.The images revived long-running speculation about Mr. Putin’s health, and about something the Kremlin has spent more than 15 years denying: that the man waging Europe’s bloodiest war since 1945 is one of the world’s most dedicated consumers of cosmetic enhancement.Days after the parade, the Kremlin released footage of Mr. Putin driving himself through the capital in jeans to collect his elderly former German teacher, Vera Gurevich, for dinner at the presidential residence.The choreographed display was presented by state media as proof that the president is firmly in command. But the moment backfired when Ms. Gurevich squinted at his face and asked if it was really him, leaving the president to swat away years of body-double rumors on live camera. By June 5, Mr. Putin was at the St. Petersburg International Economic Forum, brushing off an invitation to meet President Zelensky while Ukrainian drones hit targets outside his own city — and exiled Russian commentators were busy dissecting the performance of a man peddling optimism nobody was buying.What the mockery tends to drown out is something rather more considered. Cosmetic surgeons on three continents have spent the better part of a decade studying Mr. Putin’s face with the same forensic patience that Kremlinologists once applied to Politburo seating arrangements — and they tend to reach the same conclusion.“Reviewing images that I could find of Putin over the past two decades, his forehead and crow’s feet appeared to be treated with Botox since they are much smoother than normal for a man in his 70s,” a double board-certified plastic and reconstructive surgeon and chief medical officer at Beverly Hills Plastic Surgery, Dr. Gabriel Chiu, tells The New York Sun.What the Surgeons SeeThe visual record, Dr. Chiu notes, defies biology. As men age, their faces typically lose fat, hollow out, and sag. Mr. Putin’s has done the opposite, growing rounder and plumper from roughly 2014 onward, a change the surgeon says is “highly consistent with dermal filler injections.”Then there are the eyes. In photos from his first two terms as president, Mr. Putin carried distinctive, heavy bags beneath them, paired with sagging skin.“These under-eye bags had entirely vanished by 2011, replaced by tighter, smoother skin,” Dr. Chiu explains. “Aging would normally worsen these bags, so Putin likely had a lower blepharoplasty.”He also notes that chemical peels or laser treatments may have featured in the regimen at some point, brightening the skin and softening deeper lines.Dr. Chiu is hardly alone in his reading of the evidence. A leading British cosmetic surgeon, Gerard Lambe of Manchester’s Reflect Clinic, has assessed that the president’s widening, plumper face points to cheek fillers and a dedicated aesthetic team.A New York dermatologist and plastic surgeon, Patricia Wexler, has identified what she believes to be Botox, fillers, an eye lift, and a fat graft to address the sunkenness around his eye sockets, which appeared in the early 2000s. Toronto facial surgeons Philip Solomon and Stephen Mulholland have separately pointed to a smoothed forehead, softened crow’s feet, and added cheek volume as evidence of injectables and laser resurfacing.The paper trail is unusually public for a man obsessed with secrecy. Botox rumors first emerged in October 2010, after the then-prime minister arrived at a meeting in Kyiv sporting a large, poorly concealed bruise around one eye. A year later, Russia’s liberal New Times magazine asked what had happened to Mr. Putin’s face, quoting four plastic surgeons who agreed he had likely had forehead injections and a lower eyelid lift.The cheek fillers seemed to emerge just before the 2014 Sochi Olympics, in the period surrounding his divorce from his wife of 30 years, Lyudmila — who later took the surname Ocheretnaya. A former British foreign secretary and trained physician, Lord David Owen, has offered a competing theory, arguing the change in Mr. Putin’s facial shape points to steroid use rather than cosmetic work, noting that the president “now has an oval face.”As for the swelling on Victory Day, Dr. Chiu sees a familiar culprit.“While Putin’s noticeably puffed and fatigued appearance at the May 9, 2026 Victory Day parade could have been from weight gain, the change in the features of his face appear to be consistent with overuse of HA fillers, or ‘pillow face,’” he explains, noting that medications such as corticosteroids could also play a role but that “filler fatigue” is the likelier explanation.This refers to the cumulative distortion that sets in when hyaluronic acid injections are layered over years without allowing the face to fully metabolize prior treatments. The Strongman in the MirrorSo why would a leader who built his brand on bare-chested horseback rides and judo throws invest so heavily in the syringe? For those who study the Kremlin, the two are not in tension at all. They are the same project.“Authoritarian systems often depend on mythology; Putin’s mythology is that of a fearless strongman,” a Russia expert and research fellow at the Foundation for Defense of Democracies, Ivana Stradner, tells the Sun. “A genuinely confident leader has little need to repeatedly demonstrate his toughness.”Those shirtless photographs, she argues, cut both ways: “A man who rides shirtless to prove his strength may be revealing insecurity rather than confidence.”From Dr. Chiu’s perspective, the cosmetic maintenance likely serves Mr. Putin’s political survival directly.“In his worldview, physical vigor and an ageless appearance project strength, stamina, and eternal control,” he observes. “By erasing wrinkles and maintaining a static face, Putin projects an image of permanence. This is a subtle signal to the Russian public and the world that he is timeless, irreplaceable, and unaffected by the heavy toll of governance or war.”Ms. Stradner, however, cautions that the speculation itself can be weaponized.“Putin’s public persona is built on the promise of toughness; every wrinkle and every rumor of illness carries political significance,” she continues. “Russia’s intelligence services may well be amplifying such images in the hope that Western governments misjudge Putin’s health and resilience.”The needle and the scalpel, it turns out, are just one corner of a far larger obsession. At 73, Mr. Putin has funneled that fixation into national policy: His “New Health Preservation Technologies” initiative, unveiled in 2024 and enacted in 2025, commits roughly $26 billion to anti-aging research, including bioprinting human organs and growing them inside genetically compatible pigs.The scope of the ambition became harder to dismiss last September, when a hot microphone caught him telling President Xi Jinping that advances in biotechnology could allow human organs to be “constantly transplanted” so that people could “live younger and younger, and even become immortal.” Mr. Xi replied that humans might reach 150 years old within this century.For Mr. Putin, who reportedly swims daily, subsists on a spartan diet, and has a “particularly acute” fear of death — the phrase used by a Johns Hopkins senior fellow, Peter Pomerantsev — the fillers may be the least of it.The Kremlin’s Long War on the MirrorYet Moscow’s response to all of this has never wavered. When apparent bruises around his eyes made headlines during a visit to Kyiv in 2010, his own spokesman attributed the sight to poor lighting and fatigue, setting a template that has held ever since.The presidential press secretary, Dmitry Peskov, spent 2022 batting away questions about the president’s health. Everything was fine, he insisted, saying the rumors were the handiwork of Ukrainian, American, and British disinformation specialists. A year later, he took a different tack, addressing the body-double and bunker claims at a Moscow conference with something closer to amusement. “Yet another lie,” he said, laughing off the suggestion that a stand-in was running the country while Mr. Putin hid underground. The president, he added, was “mega-active” — so much so that the people around him could barely keep pace. “His energy can only be envied.”To Dr. Chiu, the blanket denial is itself revealing.“This reveals a desire to maintain the illusion of an untouched, rugged, and formidable leader who is the product of genetic superiority, supreme fitness, and flawless health,” he says.The regime’s intolerance of mockery goes beyond press briefings — it has been written into law. When a digitally altered image of Mr. Putin wearing lipstick and mascara, widely known online as the “gay clown” meme, spread across social media and Russian protest movements in 2017, the Justice Ministry added it to the country’s official list of banned extremist materials.The man who originally shared it online was convicted and committed to a psychiatric institution. Ms. Stradner sees the episode as “speaking volumes about how sensitive Putin is about his public image.”“Putin appears deeply intolerant of ridicule, and many Russians understand this,” she says. “Which is precisely why satire and mockery continue to target him.”
