How the Onset of Puberty Affects a Male’s Lifespan — and Why Science Has Looked Away

The question sounds deceptively simple: When did your son’s voice break? Yet that biological milestone, long dismissed as an awkward footnote in adolescence, may carry more clinical significance than anyone has publicly reckoned with.Researchers now say the timing of when a boy hits puberty may predict his long-term risk of heart disease, type 2 diabetes, or depression, and could help explain why men, on average, die younger than women.The science connecting puberty timing to lifelong male health has been quietly accumulating for years. What hasn’t accumulated at nearly the same pace is the scientific investment to address it. There were roughly 1.67 times as many published research papers on female puberty as on male puberty between 1990 and 2016, and researchers are now calling for that imbalance to be adjusted.“Early puberty in boys has been shown to increase the risk of obesity, type 2 diabetes, heart attack, depression, and attention deficit hyperactivity disorder,” the clinical director and senior investigator at the National Institute of Child Health and Human Development at the National Institutes of Health, Catherine Gordon, tells The New York Sun. “On the other hand, late puberty heightens the risk of asthma, eczema, osteoporosis, anxiety, and depression in boys and men.”So, why has medicine spent decades barely looking?Early starters, higher stakesData pulled from hundreds of thousands of men and women in the UK Biobank tells a sobering story: Males who hit puberty early faced meaningfully higher odds of developing angina, hypertension, and type 2 diabetes later in life — even after researchers controlled for other variables. These aren’t obscure or peripheral conditions. They’re the slow-building crises that land middle-aged men in cardiology units, often with no warning until it’s too late.In other words, a boy who enters puberty early may already be on a biological path toward the chronic diseases that define — and shorten — adult male life, years before any symptoms appear.A prospective birth cohort study in northern Finland tracking more than 2,400 males found that earlier pubertal onset was associated with higher adult BMI, elevated fasting insulin, higher diastolic blood pressure, and lower HDL cholesterol. In males specifically, early puberty was also linked to higher total serum cholesterol, LDL cholesterol, and triglycerides by age 31, marking the metabolic fingerprint of a man already on a deteriorating health trajectory before he hits middle age.The lifespan implications are striking. A genomic analysis published in Nature Communications found that earlier male puberty timing is associated with a shorter lifespan, and Mendelian randomization indicates the relationship is likely causal rather than merely correlational.Dr. Gordon explains that the underlying hormonal picture is complex. Both abnormally high and low testosterone levels carry distinct health risks: Low testosterone is linked to heart disease, insulin resistance, and metabolic syndrome, while excessive levels are associated with elevated blood pressure and coronary artery disease.“This has focused increasing attention on the timing of puberty in males and the accompanying rise in testosterone as a potentially important factor in lifelong health,” she continues.A professor of pediatrics at the University of Cincinnati, Frank Biro, who has studied puberty timing for decades, tells the Sun the relationship between weight and puberty in boys defies simple assumptions.“The relationship is not linear,” Dr. Biro says. “Boys with lower BMI mature later, but at higher BMI levels, boys with the highest BMIs also mature later than boys with mid-value BMIs.”Put simply: being either very underweight or severely overweight can delay puberty in boys, but for different biological reasons, meaning there is no single weight-related red flag for parents or clinicians to watch for. The risk runs in both directions, and the middle of the curve is where the earliest developers tend to cluster.The invisible risk of going lateIt is not only early puberty that carries risk. Boys who develop later face distinct vulnerabilities, which have been equally underexplored. But what defines early or late?Clinically, puberty is considered early in boys when it begins before age 9, and late when there are no signs of development by age 14. Most boys begin puberty somewhere between 9 and 14, with the process typically complete by 17. It is with tThose outside that window — on either end — that the health risks begin to concentrate.An endocrinologist at the Northern Region Gender Dysphoria Service in Newcastle-upon-Tyne, Richard Quinton, tells the Sun that boys with delayed puberty in particular are routinely dismissed by doctors who assume the delay will resolve on its own.“My own particular bugbear is how boys with delayed puberty tend to get reflexively gaslighted by doctors that they just have self-limiting delay and should just go away and forget about it,” says Dr. Quinton, who also serves as an honorary reader in reproductive endocrinology at Imperial College London.The doctor says that approach may work for younger adolescents but fails older ones, especially those with additional indicators such as a lack of sense of smell, impaired hearing, or a history of surgery for undescended testes or cleft lip or palate.“You really need to be assessed by a specialist if you haven’t started puberty by age 14, or haven’t completed it by age 16 to 17 years,” he explains.The challenge in studying male puberty is partly methodological. Unlike girls, who have a distinct and dateable biological event in their first menstruation, tracking puberty timing in boys has historically relied on less precise self-reported markers: voice change, facial hair and testicular volume measured in clinical settings.Most previous studies assessed puberty timing using self-reported measures, leading to measurement error that complicates the data and weakens findings. The tools have been less refined, and that has compounded the research gap.“One reason female puberty has received greater research attention is that its milestones, such as breast development and the onset of menstruation, are relatively easy to identify,” Dr. Gordon notes. “Pubertal onset in boys is more subtle and difficult to document. Boys may feel uncomfortable discussing these changes with parents or health care providers, making them more difficult to study.”The health math is unforgiving in both directions. Come in too early, and a boy carries elevated lifetime odds of obesity, type 2 diabetes, heart attack, depression, and ADHD into adulthood. Come in too late, and the risks shift — asthma, eczema, osteoporosis, anxiety, and depression — with the added burden, well-documented in the research, that late-developing boys are significantly more likely to be bullied, piling social trauma on top of an already fraught biological delay.Neither outcome is benign, and the window between them is narrower than most parents or pediatricians have been taught to watch for.The environment is accelerating the clockLayered on top of biological variation is a growing body of evidence that the external environment is actively reshaping when puberty begins.A 2025 review of endocrine-disrupting chemicals and pubertal timing found no simple cause-and-effect. Bisphenol A, phthalates, and per- and polyfluoroalkyl substances don’t affect all children the same way; outcomes shift depending on when and how heavily a child is exposed, their sex, and a tangle of environmental and socioeconomic variables. For boys specifically, the picture gets murkier. Some of these chemicals appear to accelerate puberty, others seem to slow it down, and researchers are still working out what either scenario means for long-term health.Dr. Gordon tells the Sun that endocrine-disrupting chemicals are compounds commonly found in everyday products and throughout the environment — including per- and polyfluoroalkyl substances, phthalates, bisphenol A, and certain pesticides.“These substances interfere with hormone synthesis in the body and the signaling of hormonal receptors,” she says. “An endocrine-disrupting chemical can mimic testosterone in a young adolescent boy, altering glands in the brain — the hypothalamus and pituitary — to activate puberty prematurely.”Dr. Biro points out that the relationship between these chemicals, weight, and puberty in boys does not follow a single clean pathway, and some researchers will dispute even the best current findings.“There are solid physiologic reasons why I believe that relationship to be true,” he says.A 2019 review in the Journal of Research on Adolescence explicitly identified boys as one of four understudied populations in puberty research, calling for a new era of transdisciplinary work. That call has not yet produced the volume of investment the science demands.Dr. Gordon highlights that the consequences of continued inaction are measurable, not just for individual men but for the broader health care system.“Being aware of heightened health risk factors because of premature or late puberty onset can help care providers address clinical risks up front and take actions to prevent or minimize their impact,” she adds. “A better understanding of how pubertal timing influences future health outcomes could ultimately transform the way we identify risk, monitor development, and intervene earlier when needed.”
